Case Management Services

True Direct Home Health Care Case Managers assist adults, children, and families to determine their needs and gain access to comprehensive services to maintain independence in their homes and communities. By linking people to services and monitoring those services, our Case Managers assist you in your recovery journey.

True Direct Home Health Care is currently providing assistance in getting enrolled in the following programs in the state of Pennsylvania:

• Aging Waiver
• Attendant Care/ Act 150
• Independence Waiver
• COMMCARE Waiver
• OBRA Waiver

For those that qualify for these programs, services may include:

• Service Coordination
• In-Home Personal Assistance (Such as bathing, meal preparation, household chores, grocery shopping, laundry, light housekeeping and companionship)
• Transportation Services
• Home-Delivered Meals
• Personal Emergency Response System
• Durable Medical Equipment
• House Modification Programs
• Adult Daily Living Services
• Health Insurance Counseling
• Respite Care
• Adult Protective Services
• Legal Assistance
